Making the Right Care Plan
We know families looking for caregivers have many things to consider and want to make the right decision for their loved ones. Our free IN-HOME ASSESSMENT can help you choose the best senior care option for your loved ones. This can be a significant first step to starting the planning process.
How Does A Care Consult Work?
When you meet with our Care Manager – here’s what you may experience:
The Care Manager will ask many questions to discover what duties and responsibilities you are expecting from our caregivers.
Reviewing the ADLs
Next, the Care Manager will ask questions about everyday Activities of Daily
Living: Bathing, dressing, cooking, and transferring in
and out of a chair or bed – those activities. Are they still manageable, or do they require assistance from our caregivers?
Identifying the need for the Support
Once the Care Manager has assessed the situation, they will explore care
options the family may benefit from,
based on any gaps or deficiencies noted.
A home tour will help identify environmental safety concerns, such as trip hazards, lack of grab bars in
wet areas, misuse of extension cords or space heaters – or any other safety concerns.
Emotional and Social Assessment
Next, the Care Manager will identify any physical or emotional risks associated with isolation and loneliness.
Care Coordination Plan
The Care Manager will then discuss any health concerns, medication
management, and other issues that
should be addressed in a care coordination plan.
Finally, the Care Manager will review the results with you and make
you can discuss the hours and types of care that would best support your family.
